ANNUAL SALARY: $55,000 – $60,000
JOB SUMMARY:
Step into a leadership role that directly strengthens the wellbeing of pregnant individuals, new parents, and families with young children. As the Case Management Supervisor, you’ll lead and inspire a dedicated team of case managers, ensuring they deliver exceptional, client‑centered support that advances our mission to improve maternal and child health. You’ll shape program quality through coaching, training, and thoughtful program development, while overseeing caseloads, service delivery, and compliance. ESSENTIAL DUTIES:
- Supervises Connecting NJ (CNJ) Case Managers with case management, care coordination, and data collection.
- Maintains direct oversight of case managers’ participation and coordination with Dept of Children Protection & Permanency (DCP&P) Plans of Safe Care meetings.
- Completes training for case management and training required by Dept of Children and Families.
- Provides regular 1:1 supervision with case managers.
- Monitors case manager growth/development by locating and providing relevant training opportunities.
- Supports case managers’ development and implementation of care plans for their clients.
- Monitors data collection by the case management team and submits monthly/quarterly progress reports addressing process and outcome indicators that will help determine strengths and areas needing improvement.
- Works closely with Early Childhood Systems of Care programs to help case managers connect to appropriate and available services.
- Utilizes skills in communication, marketing, and public relations, as well as being able to perform professional oral and written tasks (e.g., public speaking, grant writing).
- Works closely with CHW program to coordinate the flow of clients between the two programs.
- Coordinates with CNJ Supervisors to support the flow of clients within the CNJ HUB.
JOB REQUIREMENTS:
- Master’s degree in social service field (i.e., social work, public health, psychology, etc.) preferred; Bachelor’s degree required.
- Minimum 3 years of experience supervising staff
- Minimum 5 years of case management experience in human services in the public sector
- Experience interacting with human service providers and agency leaders
- Excellent organizational skills and attention to detail.
- Excellent time management skills with a proven ability to meet deadlines.
- Strong analytical and problem-solving skills.
- Strong supervisory and leadership skills.
- Ability to prioritize tasks and to delegate them when appropriate.

- High level of familiarity with county health and social services
- Establish trusting relationships and work with culturally diverse populations.
- Proficient with Microsoft Office Suite.
