Case Manager- Connecting NJ

CLASSIFICATION:  Full-Time/Non-Exempt

LOCATION: Newark, New Jersey

WORK HOURS: 37.5 hours /week

JOB SUMMARY: Responsible for coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centered. The CNJ Case Manager has a community focus on pregnant people, caregivers (mothers, fathers, grandparents, kinship, foster parents, legal guardians), and children.

ESSENTIAL DUTIES:

  • Link families to existing resources in the community and provide personalized care coordination to ensure they are connected to appropriate resources, receive timely information, and are provided continued support, advocacy, and follow-up weekly.
  • Support individuals who are low-income or uninsured, with chronic health conditions, with multiple social and economic stressors, underserved immigrants, victims of domestic violence, individuals impacted by mental health issues, alcohol or substance use disorder; and are involved with the Division of Child Protection and Permanency.
  • Carry a small caseload of 10-15 cases. Not to exceed 25% of the FTE position.
  • Complete training for case management as required by the Department of Children and Families.
  • Provide linkages and referrals to services, with special emphasis given to families requiring a plan of safe care. Case Managers will also attend monthly Plans of Safe Care meetings.
  • Meet with clients and families in-person or virtually as required.
  • Support the implementation of the DCF CNJ expansion as required.
  • Complete required documentation, complete accurately all necessary forms, and produce statistical reports.
  • Work closely with Early Childhood Systems of Care programs to help link families to appropriate and available services.
  • Utilize skills in communication, marketing, and public relations, as well as be able to perform professional oral and written tasks (e.g., public speaking, grant writing).
  • Demonstrate leadership and a willingness to take initiative by working independently and in a team.
  • Accept personal differences, establish trusting relationships, and work with culturally diverse populations when providing consumer care.

JOB REQUIREMENTS:

  • Bachelor’s degree in a social service field (i.e., social work, public health, psychology, etc.) is preferred.
  • Bi-lingual English/Haitian Creole required.
  • 3-5 years of case management experience in human services in the public sector.
  • Experience interacting with human service providers and agency leaders; understanding role of evaluation in social services.
  • A high level of familiarity with county health and social services is required.
  • Excellent written and verbal communication skills and computer literacy are required.
  • Ability to communicate clearly and professionally with a range of community stakeholders.
  • Experience providing outreach to women and familiarity with Union County health and social services required.
