ANNUAL SALARY: $50,000 – $55,000
JOB SUMMARY:
The Community Alignment Specialist (CAS) will focus on aligning community resources and services to meet families’ needs. The CAS will work to align people, processes, and supporting technologies within each CNJ hub to drive better outcomes for our youngest children and families. The CAS will also be responsible for developing and maintaining relationships with agencies that serve pregnant women, children (birth to five), and their families. This includes establishing reciprocal referral networks and ensuring strong community representation.
ESSENTIAL DUTIES:
- Attend all required and suggested training for Family Connects International to maintain program fidelity. Attend all required meetings and training deemed necessary by DCF and PMCH
- Develop a comprehensive knowledge and an in-depth understanding of PMCH programming, focusing primarily on CNJ and FCNJ and effectively promoting programming services to agencies, stakeholders, and community residents.
- Deliver two educational sessions monthly to enhance community awareness and engagement
- Plan and execute two large events annually to support program objectives and community involvement
- Gather and organize essential information about local services and resources to create an agency directory for the FCNJ Salesforce Database system.
- Attending weekly nurse case conference meetings to provide individual case consultations about community resources to support nurse home visitors as they connect families with community resources, as needed. Communicate regularly and as necessary to ensure access to information and services for CNJ referrals.
- Market the program by communicating with referral sources and conducting site visits to maintain strong relationships with the broader community. Work collaboratively with the county CHWs and the local County Council Young Children board to ensure that the county has an active and successful local CAB.
- Co-lead Community Advisory Board (CAB) meetings, which include, but are not limited to, taking minutes, booking speakers, tracking members, and maintaining records and reports. Facilitate CQI processes for the local hub to improve the referral process and alignment of resources.
- Monitor and track reports using the CNJLink database to ensure accurate and up-to-date information.
JOB REQUIREMENTS:
- Master’s degree in a related field (e.g., public health, psychology, social work, nursing, communications, marketing) or a bachelor’s degree with additional related experience preferred.
- Requires progressive experience in human services within the public sector, including skills in interacting with human service providers and agency leaders, and an understanding of the role of evaluation in social services. A combined total of five years of experience in a related field is desired.
- Bilingual (Spanish/English) preferred.
- Two years of social service experience and computer literacy required.
- Excellent written and verbal communication skills, with strong attention to detail, are required.
- Familiarity with county health and social service resources required.
