JOB SUMMARY:
- JOB SUMMARY: Work within the local community and lead agency structure to ensure that the Family Connects NJ Program for postpartum nursing home visits is implemented in accordance with established guidelines, model fidelity, and contract requirements in Hudson County. Use principles of supervision, reflective practice, and staff development to provide primary support and oversight to a team of nurse home visiting staff.
ESSENTIAL DUTIES:
• Provide direct weekly reflective supervision and evaluation of 8 nurse home visitors, Nurse Lead, and administrative staff.
• Participate in the Family Connects International model required training and participate in ongoing training webinars and professional development trainings.
• Perform Quality Assurance assessments and quarterly fidelity field visits with each nurse home visitor.
• Lead Monthly Quality Assurance meetings with internal and external stakeholders.
• Assign client caseloads and monitor home visitor’s ability to complete home visits and to complete necessary documentation, following the Family Connects International model.
• Maintain a caseload of up to 4 clients as needed and conduct in-person visits with families.
• Lead weekly case conferences with nurse home visitors in fidelity with Family Connects International model.
• Develop and maintain community and provider referral sources for the program, along with nurse home visitors and support staff.
• Actively participate in preparation and monitoring of program budget to ensure appropriate use of funds.
• Oversee and ensure timely and accurate data input and utilization of web-based data systems for nursing documentation.
• Effectively support nurse home visitors in analyzing complex problems and clinical case situations and consult with Medical Director as needed for clinical guidance.
• Ensure that all program and training materials are ordered in a timely manner, organized, and available for use by nurse home visitors.
• Participate in community advisory board meetings to assist in coordinating services.
• Evaluate implementing agency outcomes and interventions.
• Prepare and submit quarterly progress reports.
• Handles other assignments as requested.JOB REQUIREMENTS:
• Bachelor’s Degree in Nursing required
• Master’s Degree in Nursing or closely related field preferred
• Current NJ RN license (in good standing)
• BLS certification
• Preferred 5 years of experience providing maternal/child health service in community setting, home visitation, OB-GYN nursing, neonatal and/or pediatric nursing, labor and delivery, adult mental health, or closely related field.
• Preferred 3 years of program management/supervision experience.
• Experience working with diverse populations, communities, and organizations.
• Excellent verbal and written communication skills
• Computer proficiency in MS Office Suite
• Highly collaborative styles, skilled at relationship and consensus building
• Strong initiative and follow through with enthusiastic positive attitude
• Bilingual preferred
• Ability to work collaboratively with various agencies and stakeholders
• Multi-tasking skills a must
