Program Manager     

CLASSIFICATION:  Full-Time 

LOCATION:  Irvington, New Jersey

WORK HOURS: 37.5 hours/week

SALARY: $65,000 – 70,000/year

JOB SUMMARY: Responsible for developing and maintaining relationships with agencies that serve women of childbearing age for the purposes of establishing reciprocal referral networks and generating representation on community advisory boards and coalitions. Supervises a team of Community Health Workers and offers guidance to ensure education and outreach materials and services are clinically accurate and represent best practices.

ESSENTIAL DUTIES:

  • Works closely with the Senior Manager of Community Programs, other HWHF staff, and their Connecting NJ and CNJ-Expansion hubs within the region and the municipalities that are part of that region.
  • Support overall HWHF CHW unified program goals in collaboration with the Senior Manager of Community Programs.
  • Attend weekly team supervision meetings with Senior Manager of Community Programs and Director of Community Programs
  • Seek out support from the Senior Manager of Community Programs and meet regularly to discuss program updates.
  • Train a team of 7 Community Health Workers, including supporting their assessment, linkage, and case management activities in collaboration with the Senior Manager of Community Programs.
  • Provide direct 1:1 supervision to CHWs engaged in case management activities, ensuring a minimum caseload of 25
  • Provide supervision and support to CHWs to conduct outreach, networking, and education (20% of their time) and case management (80% of their time).
  • Support CHWs in conducting outreach, networking, education (20% of their time), and case management (80% of their time).
  • Ensure CHWs refer clients to the Postpartum Doula initiative, Family Connect New Jersey (FCNJ) and connect clients to breastfeeding education activities.
  • Participate in daily case conferencing for covered counties for high risk and DCP&P referrals.
  • Develop and maintain relationships to ensure that CHWs can provide outreach to a wide array of high-needs individuals, with particular emphasis on locations that serve women not yet engaged in mainstream service systems.
  • Review and provide oversight for CHW case management and assessments and conduct program evaluations using the Unite Us and CNJ database systems in collaboration with the Senior Manager of Community Programs.
  • Assist in integrating the Community Health Worker program into Union and Essex coordinated care systems, including clinical care and EBHV.
  • Participate in Finance Budget planning.
  • Serve as the representative of the HWHF Program for Essex and Union on CNJ Community Advisory Boards and attend Quarterly DCF-DOH meetings.
  • Complete all reports and respond to all requests required by the funder and external evaluator.
  • Performs other duties as assigned.

JOB REQUIREMENTS:

  • Bachelors or Master’s in Social Work (Master’s Preferred).
  • Minimum two plus years program management and supervisory experience, computer literacy, and experience in nonclinical environments required.
  • Excellent written and verbal communication skills required; especially required is the ability to communicate accurate clinical information at a low health literacy level.
  • High level of familiarity with New Jersey’s health and social service resources required.
