ANNUAL SALARY: $55,000 – $60,000
JOB SUMMARY:
The Case Manager Program Supervisor leads the implementation and oversight of the Connecting NJ expansion across Hudson, Union, Passaic, Bergen, and Morris counties. This pivotal role ensures effective management of case managers and drives success through strategic supervision, program execution, and robust data collection to support continuous evaluation and improvement.
ESSENTIAL DUTIES:
- Oversee Connecting NJ (CNJ)-CM Expansion Case Managers with case management, care coordination, and data collection.
- Maintain direct oversight of case managers’ participation and coordination with the Department of Children Protection & Permanency (DCP&P) Plans of Safe Care meetings.
- Complete training for the case management expansion required by the Department of Children and Families (DCF).
- Support Case Managers with access to necessary training
- Provide weekly 1:1 supervision with case managers.
- Support case managers’ development and implementation of care plans for their clients.
- Monitor data collection by the case management team and submit monthly/quarterly progress reports addressing process and outcome indicators that will help determine strengths and areas needing improvement.
- Work closely with Early Childhood Systems of Care programs to help case managers connect to appropriate and available services.
- Utilize skills in communication, marketing, and public relations, as well as be able to perform professional oral and written tasks (e.g., public speaking, grant writing).
- Demonstrate leadership and a willingness to take initiative, working both independently and in a team.
- Work closely with the CNJ, ECS and CHW programs to coordinate the flow of clients to case management and assignment post-case management.
- Oversee Case Managers receiving referrals from and referring to Family Connects NJ (FCNJ).
- Work closely with the CNJ Supervisors to support the flow of clients within the CNJ HUB.
- Attend all DCF-specific meetings.
- Work closely with the Finance department on monthly budgets, variance reports, and spending.
- Supervisor is responsible for hiring case managers, as well as for planning coverage/transition plans for vacant positions
- Handle other duties as assigned.
JOB REQUIREMENTS:
- Master’s Degree in a social service field (i.e., social work, public health, psychology, etc.) is preferred.
- Minimum 3 years of experience supervising staff
- Minimum 5 years of case management experience in human services in the public sector
- Experience interacting with human service providers and agency leaders
- High level of familiarity with county health and social services
- Excellent written and verbal communication skills and computer literacy are required.
- Ability to communicate clearly and professionally with a range of community stakeholders.
- Establish trusting relationships and work with culturally diverse populations when providing care to consumers.
