Family Connects NJ, a program by the New Jersey Department of Children and Families, is expanding to Bergen, Hudson, Passaic, and Sussex counties in northern New Jersey in 2025. In this informational webinar for healthcare professionals, Emily Haines, MBA, BSN, RN, the Partnership’s Chief Nursing Officer, was joined by Regional Directors Siomara Diaz, MSN-NE, RN, CCM, and Jessyca Ramos, BSN, RN, CBC, for a live webinar to share an overview of this innovative postpartum nurse home visitor program.
The Partnership began serving Essex County residents in January 2024. The initiative has been very successful, with over 700 home visits completed to date. The webinar highlights findings from FCNJ in Essex County, outlines the referral process, and provides a road map for the January 2025 expansion.
By 2027, all New Jersey residents will have access to this innovative program aimed at improving maternal health outcomes.
