Family Connects NJ Health Professinals
Family Connects NJ is an evidence-based program that connects parents with a specially trained nurse for a personalized home visit within the first two weeks after their child’s birth. Visits are offered to ALL families welcoming a newborn in the State of New Jersey.
Healthcare providers are essential partners in helping families access this resource. Your recommendation is a key influence in a family’s decision‑making, and your referral can ensure they receive the guidance and reassurance they need during a critical developmental window.
Evidence from randomized controlled trials shows that families participating in Family Connects experience more frequent positive parent‑infant interactions, stronger community support networks, lower rates of postpartum clinical anxiety, and fewer emergency medical visits for infants.
Family Connects NJ nurse visits have been rolling out in NJ counties incrementally and will continue until they are available statewide. As of 2026, all New Jersey counties are being served by Family Connects NJ, with the exception of the final four counties of Union, Hunterdon, Morris, and Warren, which will launch their services in 2027. Family Connects NJ is funded by the New Jersey Department of Children and Families.
