Family Connects NJ
All parents of newborns can use a little extra support from a registered nurse! Family Connects NJ is an evidence-based program that connects parents with a specially trained nurse for a personalized home visit within the first two weeks after their child’s birth. Visits are offered to ALL families welcoming a newborn in the State of New Jersey.
New Jersey families who have welcomed a newborn into their lives, whether through birth, adoption, or as a resource parent or kinship caregiver, can receive the support they may need as they navigate the challenges, joys, and adjustments of parenthood. Whether this is your first child or one of many, every parent can use a little extra support, and Family Connects NJ will be there with a helping hand to ensure you and your baby are off to a healthy and confident start.
The first weeks after childbirth are crucial for both your health and your baby’s well-being. During the Family Connects NJ visit, nurses assess the health of both parents and the newborn. Additionally, Family Connects NJ provides resources and support to help parents with the transition, helping families navigate this critical time with confidence and care.
Family Connects NJ nurse visits have been rolling out in NJ counties incrementally and will continue until they are available statewide. As of 2026, all New Jersey counties are being served by Family Connects NJ, with the exception of the final four counties of Union, Hunterdon, Morris, and Warren, which will launch their services in 2027. Family Connects NJ is funded by the New Jersey Department of Children and Families.
